超声乳化白内障吸除联合小梁切除术与联合房角分离术治疗原发性急性闭角型青光眼合并白内障的临床效果和安全性分析(1)
【摘要】 目的 探讨超声乳化白内障吸除联合小梁切除术与联合房角分离术治疗原发性急性闭角型青光眼合并白内障的临床效果和安全性。方法 83例原发性急性闭角型青光眼合并白内障患者， 根据手术方法不同分为对照组(41例)和实验组(42例)。对照组患者采取超声乳化白内障吸除联合小梁切除术进行治疗， 实验组患者采取超声乳化白内障吸除联合房角分离术进行治疗。比较两组患者术后视力水平及并发症发生情况。结果 实验组患者术后视力水平优于对照组， 差异具有统计学意义
(χ2=7.179， P<0.05)。实验组患者并发症发生率为4.76%， 低于对照组的19.51%， 差异具有统计学意义(χ2=4.260， P<0.05)。结论 原发性急性闭角型青光眼合并白内障患者行超声乳化白内障吸除联合房角分离术， 效果优于超声乳化白内障吸除联合小梁切除术， 可有效的提高患者视力水平， 降低并发症发生率， 安全性更高， 值得在临床上推广应用。

【Abstract】 Objective To discuss the clinical effect and safety of phacoemulsification combined with trabeculectomy and phacoemulsification combined with goniosynechialysis for primary acute angle-closure glaucoma complicated with cataract. Methods A total of 83 cases of primary acute angle-closure glaucoma complicated with cataract were divided into control group (41 cases) and experimental group (42 cases) by different surgical methods. The control group was treated by phacoemulsification combined with trabeculectomy， and the experimental group was treated by phacoemulsification combined with goniosynechialysis. The postoperative visual acuity and occurrence of complications were compared between the two groups. Results The postoperative visual acuity of the experimental group was better than that of the control group， and the difference was statistically significant (χ2=7.179， P<0.05). The incidence of complications of the experimental group was 4.76%， which was lower than that of the control group 19.51%， and the difference was statistically significant (χ2=4.260， P<0.05). Conclusion Phacoemulsification combined with goniosynechialysis for primary acute angle-closure glaucoma combined with cataract is better than phacoemulsification combined with trabeculectomy， and it can effectively improve visual acuity， lower the incidence of complications with higher safety， which is worthy of clinical promotion and application.

白内障主要多发于老年人群， 属于较为常见的一种眼科疾病， 会对患者的视力造成不同程度的影响， 且白内障与青光眼也是导致视力丧失的2個病因。目前， 全球约4500万白内障患者中， 导致其致盲的约占46%， 属于全球第一致盲眼病。临床上首选手术治疗该疾病， 但手术的方法不同， 其治疗效果也不同。治疗白内障的手术方式有白内障囊外摘除、白内障超声乳化术, http://www.100md.com(刘芳 熊国平)
